The Northeast Youth Hockey Association Huskies offer Learn-to-Skate Hockey, Mites, Squirts, Pee Wee, Bantam, and Midget youth hockey groups at the University of Connecticut's Mark Edward Freitas Ice Rink.
I hereby accept a position with the Northeast Youth Hockey Association (NEYHA), Northeast Huskies on the xxx Travel Team for the 2018/19 Connecticut Hockey Conference (CHC) season. We, the undersigned player and parents/legal guardians of said player, by executing this Letter of Commitment, agree to the following terms and conditions of this commitment:
We agree to remain with the program for the duration of the 2018/19 hockey season. If we decide to leave, we are still obligated to pay the full fee for the season.
We understand that NEYHA reserves the right to add players in any position to rosters, as it deems appropriate in accordance with USA Hockey and CHC policies and procedures.
We understand and agree that commitment is to NEYHA and not a specific coach. NEYHA will make every effort to provide qualified coaches with the appropriate certifications.
We agree to pay in a timely manner the full season financial obligation, as set forth by the above named Connecticut Hockey Conference member program. We understand and agree that there are no refunds and that the fees for the entire season are due and payable upon NEYHA payment schedule upon signing this document.
We understand and accept that if the above named player refuses to participate with the above named program that he/she is ineligible to participate with any other Connecticut Hockey Conference member program and must sit out the remainder of the current season.
We understand and accept that a player who sits out due to refusal to participate remains obligated to satisfy any outstanding financial obligations due to the program for the 2017/18 season.
We understand and accept that a player who sits out all or a portion of a season is not entitled to any partial or full refund of program fees or expenses.
We understand and accept that player must obtain a proper Connecticut Hockey Conference release from NEYHA, and satisfy any financial obligation to NEYHA, as well as returning any property that belongs to NEYHA prior to registering with any new program in any ensuing season.
We agree to abide by all the Rules and Regulations and Codes of Conduct set forth by NEYHA, USA Hockey, and CHC. Failure to do so could result in suspension or expulsion from participating in all NEYHA practices, games, and events. We also recognize that refunds are not provided as a result of disciplinary actions taken by the NEYHA Board of Directors.
We recognize that hockey is a dangerous sport that could result in injuries and long-term illness. We understand that we are still obligated to satisfy the financial obligation regardless of the time missed. Per our Payment Policies and Procedures, members have the right to petition the Board in regard to injuries and the Board would have the right at its sole discretion to review on a case-by-case basis.
NEYHA understands and agrees that if the above named player is accepted to and commits to a Tier 1 program within CHC by April 30, 2018 this contract will be void and the commitment fee (not the assessment/registration fee) will be returned.
We understand that accepting a commitment to a travel level team means that we are making a commitment to the coaches and teammates and we will make every effort to attend all NEYHA team events.
We understand that if our skater is registered with a non-CHC program that NEYHA team events are to be prioritized. And that we are expected to attend a minimum of 75% of the games to be eligible to participate in the CHC state tournament at the end of the season.
Payment of a non-refundable commitment fee of $300 is either attached or already paid via PayPal through the NEYHA web site. For all players the signed commitment letter and fee is due by April 15, 2018.
*We, the undersigned player and parents/legal guardians of said player, further understand and accept that this Letter of Commitment may only be terminated by mutual consent between the above named program and player. Upon such mutual consent, and payment of any outstanding financial obligations, the program will issue a proper Connecticut Hockey Conference Release and the player may transfer to another program. Such mutually agreed on transfers must occur prior to November 1, 2018.
